When a parent or legal guardian will not be in attendance at a child’s appointment for vaccination(s), including the nasal flu vaccine, written consent from a parent/legal guardian is required.
Please complete our parental consent form, indicating which vaccines you consent for our team to administer, to present to the Nurse at the time of your child’s appointment.
